Fauquier Health Home Care Services has announced that it has joined a larger network of home care service agencies across the nation. This new partnership will allow its team to enhance and expand their services to better meet the needs of people throughout the region.
Effective January 1, 2017, Fauquier
Health Home Care Services transitioned ownership and management to a joint
venture of LifePoint Health (Fauquier Health’s parent company that also
partners with Fauquier Hospital) and LHC Group, a leading post-acute care
provider offering home health and hospice care, long-term acute care and
community-based services. This joint venture will operate all of LifePoint’s
home health agencies and hospices nationwide. The innovative new partnership between
LifePoint and LHC Group brings together the capabilities of two experienced
healthcare organizations and will enable the development and expansion of home
health services throughout the communities they serve.

comforting surroundings of the place they call home.”
Under the new ownership model,
there will be no interruption in service or in the timeliness or quality of
care provided by the Home Care Services professionals at Fauquier Health. The
agency will continue serving patients in its
service area from its current location at 170 West Shirley Avenue, Suite 101 in
Warrenton. There will be no changes in contact information; patients and
physicians will continue to call 540-316-2700. Fauquier Health Home Care
Services will continue to operate under local management and the same nurses
and therapists will continue to see patients. Maria
Connerley, RN, has been named the new director of nursing.

LHC Group maintains 133 hospital
and health system partnered home health and hospice locations, and more than
300 post-acute care locations across 26 states. LifePoint currently operates 20
home health and 10 hospice locations within its network of more than 70
hospitals in 22 states.
